This is a simple checklist for you to make sure your portfolio is complete.  You can use the same piece of evidence for more than one of the items on the list.  If you have any questions or would like some help with identifying suitable evidence for your HLP portfolio, please do get in touch so we can help you. Email LPC@communitypharmacyss.co.uk or call 01372 417726.

This can be used to record the one piece of evidence you need for each item on the checklist.  You do not need to record anything next to the RPS text – this is for guidance only.

This needs to be completed and kept in your portfolio.  If you need additional guidance please contact LPC@communitypharmacyss.co.uk

Joint Strategic Needs Assessment

The Joint Strategic Needs Assessment looks at the current and future health and care needs of Surrey’s residents to inform the planning and buying of health, well-being and social care services. The Health and Well-being Board has joint responsibility for overseeing this.

 Finally a top tip:

When you are ready to make your online declaration to the RSPH go to:

Also, please remember that it takes up to 2 weeks to get email confirmation of your HLP level 1 accreditation after completing the RSPH declaration online… so it MUST be done at least 2 weeks before the 24th November Quality Payment Scheme Review Point for your completion of the Quality Point Declaration to NHSBSA.

Given the feedback at the event that some pharmacies have not received a confirmation email from RSPH due to the site crashing, please do chase this a few days after completing your online declaration.  The telephone number is 020 7265 7322.

There is also a list of FAQs on the RSPH HLP level 1 Declaration webpage.
