Public Health Local Service Agreements 2018/19 – How to sign up

This information is for community pharmacies who wish to renew their contract to continue to provide public health services and for pharmacies who may wish to sign-up to provide public health services – commissioned by East Sussex County Council.

Some pharmacies/multiples will arrange for this to be done centrally, please check with your company managers/head office to find out the company procedure for your pharmacy.

Please find below the link to East Sussex County Council (ESCC) Public Health Local Service Agreement (PHLSA) Contract and Service Specifications for services to be delivered from July 2017:   Chlamydia Screening, C-Card, EHC, Needle and Syringe Exchange, Supervised Consumption and Stop Smoking.

Please sign and return the overarching contract along with the appropriate signature page for any of the service contracts that you wish to deliver. Remember to provide evidence of accreditation or training requirements where requested.

Two copies of each contract must be signed and returned by post to:

Public Health, East Sussex County Council, East C, County Hall, St Anne’s Crescent, Lewes BN7 1UE

For ease of signing you will see that ESCC have produced a separate sign-up sheet – ‘PHLSA COMMUNITY PHARMACY SIGN UP SHEETS 2018/19’. This contains all the sections that require a signature.

Please ensure you return all relevant sign-up sheets by Friday 15th June 2018 This is to ensure that the information is transferred to PharmOutcomes ready for you to deliver services from the beginning of July.
