Transfer of Care around Medicines (TCAM) Launch – Surrey

The Surrey Heartlands TCAM pathway goes live in Surrey in October 2019.

You will hear from Kent Surrey & Sussex Academic Health Science Network along with partners across Surrey Heartlands including Ashford and St Peters Hospitals NHS Foundation Trust, Royal Surrey County Hospital, Surrey and Borders Partnership NHS Foundation Trust, Community Pharmacy Surrey and Sussex and also Pinnacle Health who will be demonstrating how PharmOutcomes supports the new pathway.

Places are limited so book now to secure your place. Registration starts from 6.30pm with drinks and a light buffet served on arrival. Tickets are FREE, but must be booked in advance.

Keeping patients safe when they transfer between care providers

Health professionals know that poor communication about medicines when a patient transfers from secondary to primary care is not new and the transfer of care process is associated with an increased risk of harm. It’s something that has frustrated hospital pharmacies, community pharmacies and GP’s for a long time, this is what the Transfer of Care Around Medicines (TCAM) programme aims to address.

When patients are discharged from hospital, they may be referred through PharmOutcomes, a safe and secure digital platform, for advice from their local community pharmacist on how to manage their medicines with confidence. By improving the transfer of information about a patient’s medicines should help ensure they continue to take the right medication and reduce the incidence of avoidable harm.

The TCAM pathway also brings benefits to Community Pharmacy through better informed dispensing and operational processes, and a reduction in waste medicine.
